Gladys Russell married a duke she didn't choose, and The Gilded Age opens its fourth season by handing that duke the worst choice of his life.

The Gilded Age is Julian Fellowes' HBO drama about old and new money in 1880s New York. Gladys (Taissa Farmiga) is the daughter of railroad tycoon George Russell (Morgan Spector) and his wife, Bertha (Carrie Coon), who spent the third season pushing her into a marriage with Hector Vere, Duke of Buckingham (Ben Lamb). Gladys married him, moved to England and, at the end of season 3, announced she was pregnant. Season 4 premiered on November 1, 2026, with "Ring the Bells."

Farmiga has a short summary of her character's year.

"I was thinking. This poor girl does not get a break," Farmiga told Deadline.

The choice

The episode opens in the middle of Gladys's labor in England, with Bertha at her side. It's going badly. Bertha tries to give her a remedy with morphine in it, and the doctor sends her out of the room, because it could kill the baby.

Then he finds the baby is breech, and he tells the family he may not be able to save both. Hector's sister, Lady Sarah Vere (Hattie Morahan), wants the baby saved: the heir comes first. Bertha wants her daughter. Hector decides, and overrules his sister:

"Save them both if you can. But if a choice must be made, then save the Duchess."

"In that time, the heir was everything," Coon told Decider.

Does Gladys die?

No. The doctor saves them both, and Gladys gives birth to a healthy boy, the heir Lady Sarah wanted.

It almost goes wrong anyway. Gladys seems to fall asleep after the birth, and her maid, Adelheid Weber (Erin Wilhelmi), notices she's hemorrhaging. The doctor stops the bleeding the only way he can, by removing her uterus. Gladys survives, and she will never have another child.

The secret

Nobody asks Gladys about the surgery, and nobody tells her about it afterward. Bertha persuades Hector to wait until Gladys is stronger, so she can enjoy her son first. The doctor leaves it to Hector to decide when she finds out.

That was standard for the 1880s. Doctors talked to husbands, and a married woman's medical information went through hers. Coon has her own word for it.

"It's wild, the gatekeeping of information," Coon told TVLine.

What Bertha does tell Gladys is the good part: when it came to a choice, Hector chose her. Gladys takes it as proof her husband loves her. She still doesn't know that the son she just had is the only one she will ever have.
