Redditors have recently recalled presumably the most intriguing hint dropped by the movie’s director, the industry’s icon Martin Scorsese. It turns out that throughout the whole movie it’s shown that every time DiCaprio's Teddy smokes cigarettes, he gets them lit by somebody else, most often by Mark Ruffalo ’s character, his partner Chuck.

What’s the matter with it, you may think, but it’s a major clue to solving the movie’s mystery.

And the truth is that it “foreshadows that he is actually a patient as mental patients are not allowed to have matches,” reveals Redditor @rasterblaster1111.

Indeed, it perfectly correlates with Shutter Island’s finale, when DiCaprio's character realizes he was never Teddy, but Andrew Laeddis, who murdered his own wife after she drowned their three children. Thus, he’s being held in the facility and put in an elaborate role-play to regain his true persona and to heal his post-traumatic delusions.

There are also other clues pointing to it, for example, depicting the armed guards in the background whenever Teddy is in the focus, as voiced by @LunarsphereTapestry, and so on. The whole movie turns out to be a curious puzzle box for the viewers to solve.
