Why is Martin afraid of blood in Best Medicine? The reason he can't even take Louisa's blood goes back to Boston, not Port Wenn
A Harvard-trained heart surgeon with hemophobia is a premise, not an explanation. Fox's series takes two seasons to work backward from the symptom, and the answer isn't in the town he moved to.
Best Medicine premiered on Fox on 4 January 2026 — thirteen episodes through 7 April — developed by Liz Tuccillo from Dominic Minghella's Doc Martin. Josh Charles plays Dr. Martin Best, who leaves a Boston surgical career for a private practice in Port Wenn, Maine, a fishing town where he spent childhood summers. Abigail Spencer plays schoolteacher Louisa Gavin, Annie Potts plays his Aunt Sarah, Josh Segarra plays Sheriff Mark Mylow, and Cree plays his receptionist Elaine Denton.
What happened in the operating room
Martin froze mid-operation. The patient was a young girl who had been in a car accident, and he could not continue. That ended his run as a heart surgeon and sent him to Maine.
The freeze is the event everyone in Boston would point to. It isn't the cause.

The photograph of two children
Episode 2 fills in the rest. Martin had a sister, Rosemary, who died in a car accident when they were children — which is also why he started spending every summer in Port Wenn afterward. The girl on his table in Boston was about Rosemary's age, injured the same way. Aunt Sarah is the one who treats the freeze as reasonable rather than professionally disqualifying, and the premiere hangs a photo of two kids on the wall long before anyone explains who the second one is.
The show's position is that his medical judgment was never the problem. The blood is attached to a specific death, and the operating room simply put the two in the same room.
Why Louisa is the case he can't hand off
Martin's working arrangement is that Elaine handles anything involving blood. That holds until Louisa is diagnosed with hemochromatosis, a condition managed by drawing blood on a schedule, which makes her a recurring appointment he can't quietly delegate without her noticing.
He tells her rather than keep dodging it.
"I have a hard time… with… blood, a phobia about blood, a blood phobia, if you will," Martin says in the episode, calling himself a surgeon who can't stand the sight of blood.
His stated reason for the confession is small and entirely in character: the town has started calling him "bogman," and he would rather be the doctor with a phobia than that. He admits Port Wenn has been "harder to do than I thought."
Tuccillo, speaking to TV Insider in 2026, described the earlier forced disclosure to Elaine as the worst-case scenario for him.
"I think he hates it, which is what's so great. It's like the last person he'd want to have to trust with a secret," Tuccillo said.
Season 2 takes it public
Fox renewed the show in March 2026, raising the order from twelve episodes to fourteen, and season 2 premiered on 20 September 2026. The premiere pulls the secret out of the office entirely: during a crisis at the lighthouse involving a woman named Cecilia, Martin states his phobia in front of the town, which defuses a threat from Glendon to expose him. A medical board monitor is already watching him. Most of Port Wenn takes it in stride. Sarah does not, because she found out with everyone else.
For the record: the original British series ran fourteen years and gave its doctor the same condition with a different origin, developed as a vascular surgeon in London. The American version moved the hospital to Boston and put the cause about twenty-five years earlier than that.
