Season 2

  • Denny Duquette — Izzie's fiancé, died of a stroke right after finally getting his heart transplant.
  • Dylan Young — the bomb squad leader, blown up holding live ammunition. Kyle Chandler deserved better.

Season 3

  • Ellis Grey — Meredith's mother, cardiac arrest after years of early-onset Alzheimer's.
  • Harold O'Malley — George's dad, complications after cancer surgery.
  • Susan Grey — Meredith's stepmother, died from complications of treatment for hiccups. This show.

Seasons 5–6

  • George O'Malley — jumped in front of a bus to save a stranger; the reveal ("007…") remains an all-timer.
  • Reed Adamson and Charles Percy — both shot in the Season 6 finale hospital shooting.
  • Gary Clark — the shooter, died by suicide.

Season 8

  • Henry Burton — Teddy's husband, died on the table mid-surgery.
  • Lexie Grey — crushed in the plane crash finale, holding Mark's hand.

Season 9

  • Mark Sloan — succumbed to his plane crash injuries in the premiere. McSteamy down.
  • Adele Webber — Richard's wife, heart attack while battling Alzheimer's.
  • Craig Thomas — Cristina's mentor, heart attack mid-operation, scalpel still in hand.

Season 10

  • Heather Brooks — intern, accidental electrocution.
  • Jimmy Evans — Alex's father, post-surgical complications after years of addiction.

Season 11

  • Samuel Norbert Avery — April and Jackson's newborn son, osteogenesis imperfecta. The single most devastating hour this show ever produced.
  • Derek Shepherd — hit by a truck after saving crash victims, then finished off by a hospital that skipped the head CT. Yes, the irony was intentional.

Seasons 13–15

  • Diane Pierce — Maggie's mother, breast cancer.
  • Paul Stadler — Jo's abuser, hit-and-run. Zero tears were shed.
  • Thatcher Grey — Meredith's father, leukemia.

Season 17

  • Andrew DeLuca — stabbed while chasing down a human trafficker. Giacomo Gianniotti's farewell called playing him "one of the great honors of my life."

Season 22

  • Monica Beltran — killed by the OR explosion in the premiere, but not before talking an intern through a surgery that saved a child. A legend's exit.
  • Katie Rogers — Lucas's terminally ill patient and something much more, gone in "Wrecking Ball."

The woman behind the body count

The McDreamy call is still the one fans litigate, and Shonda Rhimes has never pretended it was an accident of contract negotiations. With Patrick Dempsey leaving, letting Derek walk out on Meredith would've retroactively poisoned eleven seasons of the show's central love story.

"Derek was going to have to die in order for that love to remain honest," Rhimes said at TCA back in 2015.

And a decade later, even she looks at the graveyard with a certain awe. Revisiting her choices on a podcast this year, she admitted:

"Oh my God. I can't believe I did that."

Ma'am. Neither can we. See you at the next funeral.
